Rooted in a Value-Based Care framework, this assessment moves beyond symptom management to proactively support measurable health outcomes, maternal confidence, and long-term family stability. During intake, we assess the 8 Dimensions of Health & Wellness—Emotional, Physical, Occupational, Social, Intellectual, Spiritual, Environmental, and Financial—to identify strengths, stressors, and opportunities for holistic support. This multidimensional approach ensures care plans are personalized, culturally responsive, and aligned with each mother’s lived experience and goals. The assessment also introduces MOM Coaching (Mindset, Optimization, and Momentum)—a structured support model that provides strategic guidance, accountability, and empowerment across pregnancy, postpartum, and early parenting transitions. Together, we establish milestone mapping and implement a Milestones & Rewards Tracking System to celebrate progress, reinforce healthy behaviors, and build maternal self-efficacy. Through measurable goal-setting, care coordination, resource navigation, and continuous evaluation, the Perinatal Concierge Service prioritizes prevention, early intervention, and outcomes-driven support.
